The following documents are necessary for the further process

The Swiss civil registry offices require the following documents:

  • Full birth certificate including parents’ names. It must be in English, German, or French, or, if issued in another language, a translation into German or English must be provided. Digital birth certificates must be notarised. (not older than 6 months, in original).
  • Single applicants must submit a certificate of single status. Single applicants provide a certificate of single status, written, signed, and dated by themselves with the current date.
  • Married applicants must submit a marriage certificate; additionally, a full birth certificate of the spouse must also be provided. (not older than 6 months, in original).
  • Widowed applicants must submit a copy of the spouse’s death certificate. (sufficient copies).
  • Divorced applicants must submit a copy of the divorce decree including the date of legal validity. (sufficient copies).
  • Certificate of residence, such as an electricity bill, tax return, or property tax statement, no older than six months (translated if necessary).
  • A copy of a valid passport or national ID card.

IMPORTANT: All documents must be submitted to the civil registry office in original form and newly issued, meaning no older than six months prior to the scheduled assisted dying date.

Residents of Switzerland will also be personally informed by the Athanasios organisation about the documents required for submission to the civil registry office.

The organisation Athanasios asks you to keep in mind following points when filling in your form:

  • A valid passport or alternative identification document is required for identity verification and entry into Switzerland.
  • Current relevant medical reports in English, German, French, Spanish, Italian, or Portuguese. Reports in other languages must be translated into either German or English.
  • Single applicants must provide a handwritten declaration of civil status, issued and personally signed within the last six months prior to the planned date.

IMPORTANT: This declaration must also be signed again on site, in the presence of an Athanasios end-of-life companion on the day of the assisted voluntary death. Athanasios can provide a template upon request.
